Kadlec RN Navigator - Neurosciences/ APM *Part Time* in Richland, Washington


Kadlec has an exciting opportunity for a RN Navigator Neurosciences/ Alternative Payment Model to join our Quality Care Management team located in Richland, Washington! (F lexible work schedule).

Shift Details: Part-time, 0.5 FTE. Variable Shift (primarily days). Shifts may include weekends.


Minimum Requirements:

  • Graduation from an accredited RN program.

  • 2 years of recent clinical experience in acute and ambulatory care Neuroscience settings.

  • Current WA State RN Licensure required.

Preferred qualifications for this position include:

  • Case management experience.

  • Proven working relationship with physicians, clinic personnel, and medical center staff, with demonstrated success in relationship building.

  • Proven leadership and change initiative experience, having managed resources either directly or within the implementation of a new initiative.

  • Experience in education program development and presentation.

  • Current knowledge of alternative payment models.

Duties include, but are not limited to, the following:

Under the direction of and in collaboration with the Neurosciences Service Line Director (SLD), the RN Navigator Neuroscience/ Alternative Payment Model (APM) is responsible for navigating neuroscience patients through all phases of the care pathway (from the preoperative setting through a 90-day episode of care), and providing leadership on performance improvement (PI) teams to ensure success in APM participation and Center of Excellence (CoE) designation. The Navigator optimizes patient, staff and provider satisfaction with the CoE program, while ensuring quality and cost benchmarks are achieved. A comprehensive understanding of neuroscience best practice standards, case management principles, APMs, effective education delivery methods, continuous process improvement and data management/reporting are key to performing this role.

  • Demonstrate and sustain compliance with Kadlec Caregiver Expectations Standard of Excellence and Code of Conduct.

  • Plans, coordinates and provides expertise and consultation by overseeing the complete care of the neuroscience patient.

  • Serves as a clinical resource and 'point person' for care coordination, and continuity, overseeing pre-op surgery preparation, education, an individualized care plan, discharge plan, and post-acute follow-up:

  • Works collaboratively with health plans, finance, and facilities to develop integrated, coordinated systems of care for patients to experience connected, seamless care

  • Maintains accurate census and monitoring of neuroscience patients, including those in APM

  • Conducts a comprehensive assessment to inform appropriate risk stratification, care pathway selection and resource utilization

  • Identifies patient needs, amenities and coordinates necessary resources to administer the services

  • Participates in rounds to ensure compliance with practice standards, address discharge barriers, monitor ancillary services utilization, and conduct patient interviews

  • Serves as a professional resource for hospital departments, physician offices, post-acute partners, outreach networks and APM payer contacts to coordinate care services:

  • Develops/reviews standardized care plans with the surgeons.

  • Maintains close, collaborative and consulting relationship with all disciplines who provide and/or support patients receiving neuroscience care (e.g., nursing, case management, therapies, pharmacy, emergency room providers, patient access, inpatient rehab, volunteers, primary care and post-acute staff, etc.)

  • Serves as patient/family advocate with physicians, health care staff and/or administration.

  • Serves as an advocate for continuous process improvement and leads interdisciplinary teams toward accelerated process improvement:

  • Oversees and incorporates patient reported outcome data collection and program-approved digital interface tools

  • Actively participates in the neuroscience CoE certification process

  • Provides neuroscience Process Improvement Team leadership for ongoing quality improvement

  • Identifies areas for improvement/ conducts continuous process improvement and patient satisfaction initiatives

  • Ensures a measurement system for tracking surgical outcomes, patient satisfaction, and financial metrics

  • Prepares neuroscience cases for review.

  • Provides key statistics to the neuroscience Service Line Director. Statistics may include: clinical outcomes, patient satisfaction/survey results, LOS and readmissions/denied days, etc

  • Coordinates neuroscience Patient Advisory Council with the Coordinator.

  • Collaborates with Clinical Education to develop, coordinate and provide staff and community education:

  • Develops and/or revises service-specific policies, procedures and protocols and ensures staff are educated accordingly

  • Oversees/ coordinates educational offerings and content for patients and their care partners

  • Oversees the development and on-going editing of service-specific forms and patient education materials such as: patient guidebooks, in-room communication materials, discharge checklists and surveys, and other standardized materials specific to the service line

  • Develops/participates in public, professional education programs to further knowledge, expertise, and awareness regarding conditions that may lead to neuroscience specialty interventions and/or current best practice protocols for neuroscience care.

  • Enhances professional growth and development through literature reviews, seminars, conferences, etc. as related to neuroscience Shares learning with others.
