Kadlec RN Care Management Specialist in Richland, Washington
Kadlec has an exciting opportunity for an RN Care Management Specialist to join our growing Ambulatory Care Management team located in Richland, Washington!
Shift Details: Full time, 1.0. Day shifts.
In this position you will be:
Demonstrates and sustains compliance with the Caregiver Expectations and Code of Conduct.
Coordinates with the Primary Care Providers, and all members of the health care team to facilitate a strong transitional healthcare pathway for the patient.
Monitors patient’s healthcare pathway to ensure adherence, removes obstacles and identify progress toward desired care outcomes.
Documents in the patient record according to department policies and procedures.
Works with patients and their family members to develop a personalized healthcare pathway that meets the individual’s needs on both a short term and long term basis.
Orients and educates patients and their families about the healthcare pathway by meeting them; explaining the role of the RNCM initiating the health plan; providing educational information in conjunction with health care team related to treatments, procedures, medications, and continuing care requirements.
Identifies high risk patients to promote coordination of services and to prevent readmissions or Emergency department visits.
Strategizes with Primary Care Providers, Home Health Services, community support services, outside vendors and hospitalists to deliver appropriate health care delivery strategies for these patients.
Coordinates information and care requirements with other care providers; resolving issues that could affect smooth care progression; fostering peer support; providing education to others regarding the care coordination process.
Follows-up with patients on a regular basis and as changes to the healthcare pathway occur to ensure that the patient is in compliance.
Remains available to answer clinical questions and offer consultative advice for program enrollees.
Serves as a patient advocate as you help them navigate the Health Care Continuum.
Upon patient request, assists patient in selecting and establishing care with Kadlec Clinic specialty providers based upon clinical review, available resources and patient needs.
Operates standard office equipment such as a personal computer, calculator, fax and copier with efficiency.
Adheres to all Kadlec Clinic standard policies and procedures related to infection control.
Participates in quality improvement and evaluation processes related to Care Management.
Graduate from an accredited nursing program required.
Current Washington State RN Licensure required.
2 years Recent hospital or clinic experience.
Current American Heart Association Healthcare Provider BLS required.
Bachelor’s Degree in Nursing from an accredited educational institution preferred.
Two years hospital case management or ambulatory care management experience preferred.
Bi-lingual or multi-lingual skills preferred.
