Kadlec Regional Medical Center has an exciting opportunity for a Patient Placement Specialist to join our growing Resource Team located in Richland, Washington!
Shift Details: Part-time, 0.45 FTE. 12 Hours Variable Shift.
Graduation from an accredited nursing program
Current WA State RN Licensure
Current American Heart Association Healthcare Provider Basic Life Support (BLS)
Computer (windows environment) experience
Experience in a variety of software applications with an emphasis in word processing and data entry
Two (2) years recent, acute care nursing experience
Bi-lingual or multi-lingual skills
Duties include, but are not limited to, the following:
As a Patient Placement Specialist you remain responsible for placement of all patient transfers-in, direct admits, and Emergency Department admits to Kadlec Regional Medical Center and facilitates acceptance of patients to Providence St. Mary's Medical Center; Facilitates physician communication and participates as necessary in identifying the accepting physician; Verifies bed availability as it relates to COBRA regulations; Establishes strong relationships with key staff throughout the organization; Provides feedback to the referring physician for transfers in from other facilities. The Specialist's responsibilities are primarily administrative and the position is not expected to perform direct patient care.
Demonstrate and sustain compliance with Kadlec Caregiver Expectations Standard of Excellence and Code of Conduct
Promote teamwork, with an emphasis on timely and understandable communication of patient care information to the patient and between care providers across the continuum of care
Interact with physicians accepting and referring patients to KRMC and/or Providence St. Mary’s Medical Center usually via telephone
Take initial physician admission orders for patient placement by telephone prompting physician, as necessary, for orders pertinent to patient’s diagnosis
Place patients appropriately using unit admission criteria
Work in close collaboration with the Patient Care Supervisor (PCC), staffing assistants and Charge Nurses
Participate in and/or leads patient placement meetings
Communicate with the Emergency Department, the Hospitalists, Intensivists, and specialists as it pertains to admissions and transfers
Facilitate physician to physician communication and identification of appropriate accepting physician, as necessary for transfers-in
Verify bed availability and other resources prior to accepting patient; complying with pertinent COBRA regulations
Keep log of all inquiries for transfers-in, including admits, non-admits and denials
Provide follow-up information to providers at referring facilities regarding patient’s transfer to KRMC
Create summary reports, as requested
Participate in process improvement projects as requested
Maintain client confidentiality
Adhere to standard policies as outlined in department related to infection control, fire and safety, hazardous chemicals, personnel policies and departmental procedures
