Kadlec has an exciting opportunity for a Case Management Specialist RN to join our growing Neonatology team located in Richland, Washington!
Shift Details: 1.0 FTE, Full-time, Day shift.
Graduation from an accredited nursing program
Current Washington State RN Licensure
Current American Heart Association Healthcare Provider BLS
2 years experience in a hospital or clinic setting
Bachelor’s Degree in Nursing from an accredited educational institution.
Certification in specialty area
2 years neonatology and/or Pediatric experience
Bi-lingual or multi-lingual skills
Duties include, but are not limited to, the following:
Under the direction of the Neonatology and Pediatric Program's Medical Director and Program Administrator, the Nurse Coordinator remains responsible for the day to day support functions for the Neonatology and Pediatric teams; Serves as a liaison to physicians, staff, discharged patients and their families to facilitate safe and effective flow of patient care in a manner consistent with the Mission, Vision and Values of Kadlec Regional Medical Center; Manages patient/family inquiries and facilitates the completion of appropriate paperwork and documentation after discharge, as needed; Completes and/or Coordinates completion of paperwork requiring physician signature, as appropriate and applicable; Provides support and assistance in quality and organizational improvement activities; Provides coordination and support to expedite efficient and effective patient throughput and flow; and participates in organizational quality improvement activities.

Serves as a liaison to physicians, staff, discharged patients and their families to facilitate safe and effective flow of patient care in a manner consistent with the Mission, Vision and Values of Kadlec Regional Medical Center.
Evaluates, investigates and resolves staffing issues and concerns in collaboration with the Medical Director, Program Administrator and unit managers.
Manages patient/family inquiries and facilitates the completion of necessary paperwork and documentation, primarily after discharge.
Assumes a leadership role and acts as a resource person to staff, physicians and discharged patients.
Provides coordination and support to expedite efficient and effective patient throughput and flow.
Acts as a liaison for community providers and agencies to communicate with Neonatology/Pediatric teams to ensure a continuum of care and coordination of discharge plan.
Works collaboratively with primary care physicians, specialists and surgeons to coordinate post discharge patient needs.
Uses the nursing process to assess and coordinate an appropriate treatment plan with the Neonatologists, Pediatric Hospitalists, ARNP's, Primary Care Physician or Specialist to follow-up on new diagnosis after hospitalist reviews labs, pathology and imaging reports received after discharge.
Triages calls by answering questions from discharged patients and uses the nursing process to assess, plan and implement patient care.
Enters orders in Epic and updates patient allergies and medical history, as required.
In emergency situations, coordinates the scheduling of primary care physician appointments to assure timely follow-up in collaboration with Case Management and Guided Patient Services.
Performs chart reviews for patient inquiries, risk management, quality care management or others as needed.
Participates in organizational quality improvement activities. Assists in developing quality improvement activities.
Coordinates data collection and works collaboratively with other departments to interpret data to implement quality improvement activities.
Supports hospital wide functions through committee attendance for quality improvement activities, policy and procedure development and updates.
Demonstrates effective problem solving ability through active listening, research, data analysis and documentation.
Demonstrates accountability for decisions by effectively communicating strategies and expected results of decision.
Demonstrates ability to cope with and manage change as well as, help others adapt to changes.
Utilizes data, resources, communication and knowledge of current nursing practice to make decisions.

Utilizes active listening skills to facilitate problem solving or referral to appropriate resource.
Updates policies and procedures as needed.
